Primary Care Provider (Family / Internal Medicine), Community Health CenterA Federally Qualified Health Center providing services to the Central New York region for over 50 years is seeking a primary care provider (internal medicine or family practice) for an employed, outpatient Monday through Friday opportunity. The primary care physician provides care for an underserved population of patients with a team approach for the patient's unique and specific needs. Reviews patient history and performs physical examinations. Evaluates, diagnoses, and provides appropriate treatment and patient care. Refers patient to specialist as needed. Provide clinical diagnostics. Review all test results and recommend suitable management for the patient. Prescribe medications and formulate ongoing treatment and disease management plans. Collect and record patient information. Maintain complete and accurate medical records. Communicate effectively with the clinical team to ensure total delivery of quality care.
