FCT staff work with the family, youth, and appropriate clinical, school, and state agency providers to develop strategies that help improve the youth and family functioning. FCT program is seeking a full-time Case Manager to have a mixed caseload as an Intensive Care Coordinator (ICC), In Home Therapy Therapeutic Training & Support (TT&S) staff, and Therapeutic Mentor (TM).Intensive Care Coordinators coordinate multiple services and supports for youth with Serious Emotional Disturbance (SED).The care coordinator facilitates the development of a Care Planning Team, assists the family and Team in identifying goals and developing an Individual Care Plan and Safety Plan. The Care Coordinator works directly with the youth and family to implement elements of the Plan, and coordinates with other Team Members involved with the family to help the youth and family reach their goals.In-Home Therapy Therapeutic Training & Support worker
provides intensive and strength-based family therapy to youth and their families who have significant behavioral health needs. The TT&S, in collaboration with the Master's level IHT Clinician, provides clinical interventions in the home/community setting with the purpose of improving the youth and family's functioning in the home setting. The TT&S supports the Clinician in implementing the family's IHT treatment plan.Therapeutic Mentor
provides one to one skills-based interventions to youth with the purpose of enhancing social, communication, and interpersonal skills. The TM works with youth in their community to teach and practice skills. The TM works with a Hub (ICC, IHT, OP) to coordinate treatment planning.Responsibilities:Demonstrates a basic knowledge of and endorses the CBHI values: Child Centered and Family Driven, Strength-Based, Culturally Responsive, Collaborative and Integrated, and Continuous Quality Improvement.Maintains a mixed caseload of TM, IHT and ICC youthAs an ICC, conducts comprehensive home-based clinical assessments including the CANS; risk management, safety, and treatment/care planningAs an ICC, works with each youth/family to assemble a Care Planning Team composed of family members, their natural supports, and appropriate collateralsAs an ICC, coordinates and facilitates regular Care Planning Team meetings to develop an individualized Care Plan for each enrolled youth/familyAs an ICC, works closely with the FCT Family Partner, the family, and the Care Planning Team to implement, monitor and modify the Care Plan as needed, and serves as an advocate/liaison for the family when appropriate to access new or different services.Collaborates and communicates regularly with other key service/clinical providersAs part of the IHT team, provides weekly family, parent and child therapy sessions in the home with the goal of improving family functioning.As a TM, provide weekly mentoring sessions to youth to teach and enhance various skills.Communicates with collaterals for the purpose of treatment planning and coordination.Effectively communicates and builds strong rapport with youth, families, colleagues, and/or community members in a manner that is sensitive to their racial/ethnic/cultural/class backgrounds.Maintain client records based on role, including completing comprehensive assessments, CANS, care plans, treatment plans, intervention plans, safety plans, contact notes, and discharge summaries.Maintains program productivity standardsWorks with insurance companies to obtain service authorizationsParticipates in all required meetings, trainings, supervisions, quality management, and related activities as may be directed by FCT program management.Follows all program and CHL policies and procedures.Maintains the highest standards of ethics and demonstrates professional attitude and conductWorks assigned hours, which includes some weekday evening hours (up to 8pm)Travel is required as part of this position, which includes travel to/from families' homes, community settings, and CHL sites.Participates in on-call rotation that includes providing telephonic support to enrolled familiesQualifications:Bachelor's degree in a human service related course of study (social work, psychology, counseling or related field)Minimum of two years in clinical setting working with children, adolescents and families with behavioral, social and/or emotional difficulties.Proven ability to effectively engage youth and/or families, successfully advocate for clients and to work well with other professionals.Comfortable providing home-based and community-based services.Demonstrate strong work ethic, ability to work independently and ability to contribute to team objectivesDemonstrate strong communication and writing skills.Ability to use a computer-based information systems.Current valid US-issued driver's license and ability to provide registered and insured automobile for work-related purposesICC/TT&S/TMAll qualified applicants will receive consideration for employment without regard to race, color, religion, sex, sexual orientation, gender identity, national origin, disability or protected veteran status.We're striving to make respect a part of everything we do at UMass Memorial Health - for our patients, our community and each other.
