POSITION SUMMARY

:

Clinical Pharmacists fill orders for parenteral and enteral medications, monitor patient drug therapy, and provide drug information. Clinical Pharmacists supervise and direct support personnel. In the absence of the director of pharmacy, a staff pharmacist may be required to assume the essential responsibilities and perform the duties of the director of pharmacy.

POSITION EDUCATION/QUALIFICATIONS

:

¨ BS Pharmacy degree or Pharm.D. or equivalent required

¨ Licensed to practice pharmacy in the state of Texas required.

¨ Completed all continuing education requirements

JOB KNOWLEDGE/EXPERIENCE

:

¨ Two ( 2 ) two years of Hospital experience preferred, but not required.

¨ Twenty ( 20 ) hours of IV admixture preparation training.

¨ Requires good communication and organizational skills.

¨ Must have ability to write routine reports and correspondence.

¨ Must have ability to speak effectively, and possess strong reasoning ability and good independent judgment.

¨ Requires working with frequent interruptions

POSITION RESPONSIBILITIES

:

¨ Promotes the facility mission, vision and values by effectively communicating them to others. Considers mission, vision and values in developing services, standards and practice

¨ Adhere to instructions issued by the department's leadership and perform duties in accordance with established DHR routines, methods and techniques, and in compliance with the recognized policies, procedures, standards and practices and within the state Regulatory Acts

¨ Review prescriptions to assure accuracy, to ascertain the needed ingredients, and to evaluate their suitability.

¨ Provide information and advice regarding drug interactions, side effects, dosage and proper medication storage.

¨ Assess the identity, strength and purity of medications.

¨ Maintain records, such as pharmacy files, patient profiles, charge system files, inventories, control records for radioactive nuclei, and registries of poisons, narcotics, and controlled drugs.

¨ Compound and dispense medications as prescribed by doctors and dentists, by calculating, weighing, measuring, and mixing ingredients, or oversee these activities.

¨ Plan, implement, and maintain procedures for mixing, packaging, and labeling pharmaceuticals, according to policy and legal requirements, to ensure quality, security, and proper disposal.

¨ Teach pharmacy students serving as interns in preparation for their graduation or licensure.

¨ Advise customers on the selection of medication brands, medical equipment and health-care supplies.

¨ Provide specialized services to help patients manage conditions such as diabetes, asthma, smoking cessation, or high blood pressure. Collaborate with other health care professionals to plan, monitor, review, and evaluate the quality and effectiveness of drugs and drug regimens, providing advice on drug applications and characteristics.

¨ Monitors drug therapy regimens for contraindications, therapeutic duplications, drug-drug interactions, drug-food interactions, drug-disease interactions, allergies, and appropriateness of drug and dose

¨ Detects, assesses, and reports suspected adverse drug reactions

¨ Provides clinical consultation and clarification to practitioners

¨ Documents all clinical activities and interventions accurately and completely

¨ Assists with pharmacokinetics consult service and dosing per organizational protocol

¨ Suggests appropriate, cost-effective therapeutic alternatives to medical staff, as needed

¨ Makes appropriate dosing recommendations

¨ Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed

¨ Recommends conversion of intravenous to oral therapy when appropriate

¨ Facilitates the identification, resolution, implementation, and communication of cost-effective medication use issues in collaboration with the medical staff

¨ Keeps pharmacy equipment clean

¨ Keeps pharmacy areas clean, neat, and well organized

¨ Maintains efficient filing systems

¨ Files papers, records, and reports

¨ Enters medication orders into computer

¨ Generates medication fill lists

¨ Generates reports as needed

¨ Issues controlled substances and controlled substance records to patient care areas and maintains records as required by law and pharmacy policies and procedures

¨ Assures adequate control and documentation of controlled substances

¨ Performs controlled substances inventories as scheduled

¨ Utilizes appropriate information resources to answer drug information requests

¨ Provides accurate, adequate, and timely drug information to the professional staff

¨ Sustains formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection

¨ Assists in performing physical inventories

¨ Rotates stock to ensure use before expiration date

¨ Maintains "borrow/lend" records

¨ Documents all recalled drugs and action taken

¨ Manages support personnel

¨ Performs essential duties of the pharmacy director in his/her absence

¨ Maintains logs, records, and other required documentation

¨ Supervises and directs pharmacy support personnel

¨ Ensures pharmacy services are provided in a timely manner

¨ Compounds and dispenses pharmaceuticals

¨ Contacts prescribers regarding unclear or problem medication orders

¨ Prepares intravenous and other sterile products accurately

¨ Prepares parenteral nutrition products

¨ Prepares chemotherapy products

¨ Reads and interprets medication orders

¨ Checks orders prepared by support personnel

¨ Receives verbal and written medication orders from licensed practitioners

¨ Screens medication orders for completeness, accuracy, and clinical appropriateness

¨ Checks medication labels

¨ Checks unit-dose medication carts

¨ Supervises the intravenous admixture preparation area

¨ Obtains medication histories from patients, caregivers, and health care providers

¨ Maintains accurate and complete patient medication profiles

¨ Reconciles patient profiles and medication administration records

¨ Transcribes orders into computerized patient medication profiles

¨ Maintains outpatient/employees prescription profiles and records

¨ Participates in medication use review activities

¨ Collects and analyzes medication use data per pre-defined criteria

¨ Reviews the use of targeted medications for compliance with current pharmacoeconomic programs

¨ Checks prepackaged medications and prepackaging records
