Our client, Founded in 1990; With a long-term commitment to AIDS advocacy and established New York State's first harm reduction-based, OASAS-licensed outpatient drug treatment program., other services include Health Home care management, behavioral health, and syringe exchange programs is looking for an Nurse Practitioner Type: Full-Time Shifts : Mon- Friday, No weekends Location: New York, NY SUMMARY OF POSITION: Under the general direction of the CMO, provide low threshold suboxone and MAT for patients enrolled in the Harm reduction center. This role will require leadership duties involving clinical protocols, communication with HW leadership and other agencies including OASAS and DOH, coordination with the Harm reduction team, and other duties as needed in collaboration with the Medical Director. Functioning as part of an interdisciplinary team of medical and case management professionals, be in regular communication with Harm reduction staff and the clinical team. The NP provides comprehensive medical evaluation, education, and management for clients who present for routine, preventative, acute, and chronic disease care services at the site. The NP will coordinate all of the services necessary for health promotion, maintenance, rehabilitation, and the prevention of disease and disability. The NP will work closely with and provide guidance when necessary to the nurses, social workers, case manager ESSENTIAL FUNCTIONS: Provide low-threshold suboxone treatment Optimizes clinical workflows and troubleshoots problems, Communicates with leadership from HW as well as with other agencies including DOH and OASA Responsible for maintaining quality metrics Performs duties with professionalism and compassion using an evidence-based and harm-reduction approach Performs complete history and physical examinations in accordance with current standards of professional practice, regulatory requirements, and agency policies and procedures. Provides primary medical care to adults and manages acute and chronic conditions. Provides MAT and low-threshold Buprenorphine therapy Assesses patients' conditions and age-specific medical needs; sets goals and prescribes appropriate clinical interventions to meet those goals. Provides complete and appropriate documentation in the electronic health record regarding clinical observations, diagnoses, treatment objectives, and patients' responses to treatment regimens. Maintains patient confidentiality. Collaborates effectively with other health care providers and the harm reduction team in planning interventions to best meet patients' needs in a timely manner and to promote the continuum of care. Prescribes appropriate laboratory and diagnostic procedures related to patient's health status and facilitates appropriate treatment interventions predicated on the results of these procedures Performs direct patient care procedures including but not limited to venipuncture and in accordance with agency policies and procedures, acceptable standards of practice, and in accordance with approved delineation of privileges Participates in the development of standards of care as assigned by the Chief Medical Officer and/or Chief Operating Officer. Orders medical subspecialty consultation and evaluates for the need for hospitalizations and coordinates the transfer of the resident/patient. Provides emergency medical care as maybe required by patients. Provides patient advocacy. Participates in continuing education programs appropriate to the practice. Participates in the Quality Maintenance Program at the request of the committee. Participates in the meetings of the Medical Department. Maintains confidential case records, medical charts and files pertaining to the resident/patient. Provides health promotion/disease prevention education specific to the needs of MOUD patients Participates in the development of patient care protocols. Maintains the Quality Assurance Program that defines methods for the identification and selection of clinical and administrative problems, along with strategies for corrective action to better provide services in accordance with current standards of professional pr EDUCATION/EXPERIENCE/QUALIFICATIONS: 2 to 4 years of experience as a Nurse practitioner Experience and knowledge of managing clients with substance abuse preferred. Experience prescribing Buprenorphine preferred Licensed to practice as a NYS Nurse Practitioner. Licensed and currently registered as a Registered Nurse in the State of New York. Previous experience with an electronic health record, preferred Visual acuity and manual dexterity to interface with the computer and perform medical procedures as indicated Auditory acuity to handle phone calls and interact extensively with patients Maintains CMEs/CEUs required for licensure Maintains CPR certifications Maintains Infection Control certification.
