The University of Iowa Health Care, Department of Pharmaceutical Care, is seeking a Clinical Pharmacist to join our inpatient team. The clinical pharmacist is responsible for delivering comprehensive pharmaceutical care to the adult acute care population. This role will provide pharmacy services in intermediate care areas, including but not limited to internal medicine and cardiology. Additional cross-training to other inpatient clinical areas may be provided based on staffing needs.

Dependent on candidate preference, this position may be between 0.7-1.0 FTE. Shifts are 8 hours in length. Potential options include:

As 2nd shift, Monday through Thursday in week 1, Saturday through Tuesday in week 2, Monday through Thursday in week 3, and Saturday through Tuesday in week 4 (0.8 FTE). The schedule cycles as 4 days on, 1 day off, 4 days on, 5 days off, and then repeats. The candidate will cover holidays falling within the "on" block.Monday through Thursday, 2nd shift, + 2 weekends per month, either 1st or 2nd shift (1.0 FTE). The schedule cycles as 4 days on, 1 day off, 6 days on, 3 days off, and then repeats. The candidate will cover holidays falling within the "on" block.Rotating 7-on-7-off, 2nd shift (0.7 FTE). The candidate will cover holidays falling within the "on" block.Rotating 7-on-7-off, 2nd shift, + 2-6 additional days, 1st shift, per month (0.8-1.0 FTE). The candidate will cover holidays falling within the "on" block.Mix of 1st, 2nd, and weekend shifts (1.0 FTE, approximately 75-80% of shifts occurring as 1st shift). The candidate participates in the holiday pool.At University of Iowa Health Care, clinical pharmacists are integral to all facets of patient care, encompassing therapeutic drug monitoring, medication reconciliation, patient education, and the precepting of pharmacy learners. This position ensures the highest standard of safe and effective patient care through meticulous clinical review and verification of medication orders. Collaboration is a key component of this role, as pharmacists work daily with the interdisciplinary team, including licensed independent practitioners, medical students and residents, nurses, social workers, and care coordinators.

Position Responsibilities:

Provide Clinical and Therapeutic Drug Monitoring (TDM)Assess the appropriateness of all medication orders, including drug selection, dosage, route, and potential interactionsEnhance pharmacotherapy regimens through the application of pharmacokinetic and pharmacodynamic principlesIdentify and address drug incompatibilities, contraindications, and patient medication allergies as necessaryEmploy clinical guidelines and evidence-based practices to inform medication therapy decisionsEngage in daily multidisciplinary rounds and review patient profilesOffer drug information and medication therapy recommendations to the multidisciplinary teamMonitor patient responses to medications and suggest modifications as neededEnsure thorough clinical documentation in accordance with departmental policies and proceduresEffectively Communicate with Patients and the Multidisciplinary Team

Conduct comprehensive medication histories and perform reconciliation for all inpatientsFacilitate multidisciplinary educational sessions and patient teaching classes as necessarySupport discharge processes by managing prescription drug pending, performing reconciliations, and providing counseling for high-risk patient populationsParticipate in Ongoing Patient Care Initiatives

Develop and implement clinical practice standards, as well as medication management protocols and guidelinesReport medication errors using the institutional standard reporting systemAct as a liaison for investigational drug services, facilitating continuation of therapy during inpatient admissions as appropriateEngage in multidisciplinary initiatives and research opportunities to advance clinical practicesServe on intra- and inter-departmental committees as requiredCoordinate discharge planning activities, including referrals to patient assistance programs, financial assistance coordinators, and support with the prior authorization processProvide Educational Services

At University of Iowa Health Care, a teaching institution, clinical pharmacists play a crucial role in the education of students, residents, and multidisciplinary team members. Responsibilities in this educational capacity are as follows:

Serve as the primary or supporting preceptor for pharmacy students and residentsLead educational sessions or inservices for interdisciplinary team membersDeliver didactic lectures to pharmacy students at the University of Iowa College of PharmacyPresent at and participating in educational programs sponsored by the Department of Pharmaceutical Care, the College of Pharmacy, other UIHC or UI departments, and various state and national organizationsAct as a mentor and role model for fellow pharmacist practitioners within the department

Education Requirements:

PharmD or Bachelor of Science in PharmacyExperience Requirements:

Current pharmacist licensure by the Iowa Board of Pharmacy or eligibility to obtain licensureOne (1) year of applicable pharmacy experience is required. Years of applicable experience include the time spent completing an ASHP-accredited pharmacy residency.Excellent written and verbal communication skillsProfessional experience working effectively with individuals from a variety of backgrounds and perspectives.Desired Qualifications:

Inpatient clinical pharmacy experience is highly desirableSatisfactory completion of an ASHP-accredited PGY1 and/or PGY2 residency in pharmacyBoard of Pharmacy Specialties (BPS) certification in a relevant specialty areaBasic Life Support (BLS) certification
