Physical Therapist needed to provide one-on-one therapy care for patients in their home setting!
Primary Job Responsibilities:
Assess a person's condition and develop individualized plans of care to help people reach their goals and maximize mobility and independence.Collaborate with interdisciplinary team and colleagues to ensure a person's needs are met.Demonstrate ability to perform standard therapy measurement techniques, including ROM, strength, and balance tests.Utilize a variety of therapeutic modalities to maximize care provided to clientsRequirements:
Vermont PT licensureHome Health experience and OASIS experience a must as this candidate will case manageMcKesson/Netsmart EMR experience strongly preferredWe are looking for a candidate with no requested time off.
Position: TherapistSpecialty/Modality: Physical TherapyLicenses/Certifications: CPR/BLS (Basic Life Support), State License-Physical Therapist-Vermont, NPI NUMBERPhysical Abilities: From 50-100lbs
