Responsibilities
Attends and actively participates in weekly team meetingsProvides rehabilitation and coordinates medical needs for clientsMedication Monitoring and Administration by oral medication or needle injectionsOrders, reviews, monitors, and discontinues medications as ordered by the physicianAssists with keeping within assigned nursing budgetConducts assessments and approves plan of care and service delivery methodsPerforms competency and skills evaluations on employees who provide direct careInforms the physician and staff of changes in the client's medical or mental condition and needsWrites progress notes, summaries, and dischargesImplements the treatment/rehabilitative activities and strategiesReports significant/major incidents or accidents and notifies proper authorities of circumstances or situations that present possible health hazardsFollows consumer health plans and provides and promotes consumer choice and facilitate independenceAssists in maintaining and updating nursing, pharmacy, infection control, and health and safety policies and proceduresOther tasks as assignedQualifications
Graduate of an accredited school of NursingCurrent Registered Nurse licensure in State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hireBachelor's degree preferred, with one (1) year of home health experience preferredExcellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist
