Actual pay will be determined by experience, skills and internal equity.POSITION SUMMARY:OSF Transitional Care Hospital provides compassionate care for patients who need extra support as they transition from one care setting to the next. The hospital offers all-private rooms and houses a 29-bed acute rehabilitation unit for individuals recovering from acute injuries, impairments, or illnesses. The hospital provides 24-hour clinical care seven days a week in both rehabilitation and long-term acute care settings. The care team, led by physicians, conducts daily visits to review and assess each patient's condition, aiming to support their journey to wellness. OSF Transitional Care Hospital offers a range of specialized care services, including but not limited to Diabetes Services, IV Antibiotic and Intravenous Therapy, Palliative Care, Post-Intensive Care Syndrome, Prolonged Mechanical Ventilation, Pulmonary Care, Rehabilitation Services, Stroke and Brain Injury Care, Wound Care, and additional types of care tailored to meet complex medical needs.A Clinical Dietitian is an expert in human nutrition and medical nutrition therapy. The Clinical Dietitian assesses nutritional status of patients based on their individualized needs and goals and develops and implements corresponding nutrition care plans. The role of the Clinical Dietitian is to evaluate current nutrition status, formulate interventions, reassess for changes and record ongoing nutritional progress. The Clinical Dietitian is an integral member of the interdisciplinary team, and helping patients and their support system to ensure patient-centered care is achieved.Qualifications
REQUIRED QUALIFICATIONS:Education:Bachelor's degree in Dietetics, Food Science or NutritionLicense/Certifications:Credentialed Registered Dietitian (RD) or Registered Dietitian eligible through the Commission on Dietetic Registration (Illinois and Michigan) or must become registered within 6 months of hire.Licensed Dietitian Nutritionist eligible (Illinois)Experience:Basic Microsoft application proficiencyPrevious experience with patients in a group or individual settingBasic knowledge of medical terminology and medical billingPREFERRED QUALIFICATIONS:Licensure/Certifications:License Certified diabetes educatorOther skills/knowledge:1 year experience working with patients in a group or individual settingOSF HealthCare is an Equal Opportunity Employer.
