License: Physician(s) assigned by the Contractor to perform services covered by this task order shall have a current license to practice medicine in any State, Territory, or Commonwealth of the United States or the District of Columbia. All licenses held by the personnel working on this task order shall be full and unrestricted licenses. Physician(s) who have current, full and unrestricted licenses in one or more states, but who have, or ever had, a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action or denied upon application
will not
be considered for the purposes of this task order. Current and valid copy of their licenses(s) shall be provided or a current search conducted no earlier than 5 days of quote submission of the state’s website.
Board Certification: All physician(s) shall be Board Certified/Board Eligible by the American Board of Psychiatry and Neurology (ABPN), and be currently certified in Basic Life Support (BLS) by the American Heart Association. All continuing education courses required for maintaining certification must be kept up to date at all times. Documentation verifying current certification shall be provided by the Contractor to the VA COR on an annual basis for each year of task order performance. All physicians performing on this task order
shall keep current
the level of certification at time of award. Copy of Board Certification from the American Board of Psychiatry
and Neurology from http://www.ABPN.org/ is required or current search result conducted no earlier than 5 days of quote submission showing current certification
OR
Copy of Board Eligibility by either:
Letter from the American Board of Psychiatry and Neurology stating eligibility; or
Copies of All Successful Completion certificates from a US Accredited Psychiatry residency program.
Copy of Medical Degree shall be provided upon submission or a current search conducted no earlier than 5 days of quote submission from a physician profile search engine confirming the medical degree.
National Contract Physician Identifier (NPI): NPI is a standard, unique 10-digit numeric identifier required by HIPAA. The Veterans Health Administration must use NPIs in all HIPAA- standard electronic transactions for individual (health care practitioners) and organizational entities (s).
Credentialing and Privileging: Credentialing and privileging is to be done in accordance with the provisions of VHA Handbook 1100.19 referenced above. The Contractor is responsible to ensure that proposed physician possesses the requisite credentials enabling the granting of privileges. No services shall be provided by any physician(s) prior to obtaining approval by the VA Western NY Healthcare System Professional Standards Board, Medical Executive Board and Director. If a physician(s) is not credentialed and privileged or has credentials/privileges suspended or revoked, the Contractor shall furnish an acceptable substitute without any additional cost to the government. The physician(s) shall not begin work until final approval is obtained from the contracting officer.
Technical Proficiency: Physician(s) shall be technically proficient in the skills necessary to fulfill the government’s requirements, including the ability to speak, understand, read and write English fluently. Contractor shall make available documents that verify the physicians current and ongoing competency, skills, certification and/or licensure related to the provision of care, treatment and/or services performed for each physician. Contractor shall provide verifiable evidence of all educational and training experiences including any gaps in educational history for all physician(s) and all physician(s) shall be responsible for abiding by the Facility's Medical Staff By-Laws, rules, and regulations (referenced herein) that govern medical staff behavior.
Continuing Medical Education (CME)/ Certified Education Unit (CEU) Requirements: Contractor shall provide copies of physician(s) current CMEs as required or requested by the VA. Physician(s) registered or certified by national/medical associations shall continue to meet the minimum standards for CME to remain current .
Contractor shall report all physician(s) CME hours to the credentials office for tracking. These documents are required for both privileging and re- privileging. Failure to provide shall result in loss of privileges for the physician(s).
Training (BLS, CPRS and VA MANDATORY): Physician shall meet all VA educational requirements and mandatory course requirements defined herein; all training must be completed by the physician(s) as required by the VA. All employees who will be working on this task order and require access to VA information and VA information systems are required to complete the following training before being granted access to VA information and its systems:

Mandatory Training for Transient Staff
This training is offered through the VA Talent Management System (TMS). Each employee will be required to self-enroll for a profile on the VA TMS site by visiting https://www.tms.va.gov/plateau/user/login.jsp. Once there, they should create a profile, launch the mandatory training, and complete the content prior to starting work at the VA. Upon completion, the Contractor shall provide the CO and COR with the employee’s printed certificates of completion from the TMS. These certificates display the employee’s TMS User ID. Failure to complete the mandatory training annually, within the timeframe required, is grounds for suspension or termination of all physical or electronic access privileges and removal from work on the task order until the training and documents are complete.
Upon submission of the proposal, the contractor shall provide the physician(s) current and valid, BLS copy (front and back) issued by the American Heart Association.
Standard Personnel Testing (PPD, etc.): Contractor shall provide proof of the following tests for physician within five (5) calendar days after task order award to the COR and Contracting Officer. Tests shall be current within the past year.
TUBERCULOSIS TESTING: Contractor shall provide proof of a negative reaction to PPD testing for all physician(s). A negative chest radiographic report for active tuberculosis shall be provided in cases of positive PPD results. The PPD test shall be repeated annually.
RUBELLA TESTING: Contractor shall provide proof of immunization for all physician(s) for measles, mumps, rubella or a rubella titer of 1.8 or greater. If the titer is less than 1.8, a rubella immunization shall be administered with follow-up documentation to the COR.
OSHA REGULATION CONCERNING OCCUPATIONAL EXPOSURE TO BLOODBORNE PATHOGENS: Contractor shall provide generic self-study training for all physician(s); provide their own Hepatitis B vaccination series at no cost to the VA if they elect to receive it; maintain an exposure determination and control plan; maintain required records; and ensure that proper follow-up evaluation is provided following an exposure incident. The VAMC shall notify the Contractor of any significant communicable disease exposures as appropriate. Contractor shall adhere to current CDC/HICPAC Guideline for Infection Control in health care personnel (as published in American Journal for Infection Control- AJIC 1998; 26:289-354 http://www.cdc.gov/hicpac/pdf/InfectControl98.pdf) for disease control. Contractor shall provide follow up documentation of clearance to return to the workplace prior to their physician return.

Outpatient Provider Duties: Provide psychiatry services at the WNY VAMC’s Outpatient Clinic. Shifts will be Monday through Friday from 8:00 a.m. to 4:30 p.m. except for Federal Holidays. Physician will provide treatment/evaluation/consultations to approximately 10-12 scheduled patients per day in the regular outpatient clinic; 60 minutes for new patients and 30 minutes for follow up patients.
Inpatient Provider Duties: Psychiatrist will provide diagnostic/evaluation to approximately 7-8 patients in an acute psychiatric inpatient setting. Certification in ECT is preferred for the delivery of ECT as needed.
Physicians will be responsible for completing all documentation and closes all patient encounters in a timely manner, by the end of shift, utilizing the Computerized Patient Record System (CPRS)
Physicians will be responsible for diagnosing, treating and managing psychiatric patients according to current standards of care. Services to be performed include, but are not limited to: performing assessments/examinations on initial encounters with unscheduled, emergent, urgent, and new patients; coordinating patient treatment plans; ordering appropriate diagnostic tests; recommending patients for hospital admission; and documenting in the computerized medical record.
Standards of Care: Physician(s) services shall cover the range of psychiatric services as would be provided in a state-of-the-art health care treatment facility and the standard of care shall be of a quality, meeting or exceeding currently recognized national standards as established by:
