Responsible for the management and coordination of care management services across the continuum of care at Hannibal Regional Healthcare System. Responsible for the development and implementation of the organization's care management plan. This position facilitates an effective transition planning that supports the Hannibal Regional Healthcare System's model, quality patient care, patient safety and financial components; promoting integration of a seamless care model; coordinating patient throughput; healthcare team collaboration and coordination as evidenced by metrics, optimizing performance and adoption of best practice. Serves as an advocate for patients, assisting them to reach optimal function and health in an appropriate environment that supports their needs. Oversees social services program.
Graduate of approved School of Nursing requiredBSN requiredCurrent licensure in State of MO as a Registered Nurse or licensure eligibility requiredCertification in Care Coordination and Transition Management (CCCTM) preferred or obtained within two yearsPrevious experience in/knowledge of population health, chronic disease management, and/or transitional care requiredSupervisory/management experience preferredBLS validationKnowledge of Regulatory Agency RequirementsFamiliar with Basic Nursing Performance Standards as outlined in the MO Nurse Practice ActStrong communication and leadership skillsAbility to manage multiple prioritiesMust have ability to interact effectively with a variety of people and situations at all levels of the organization required
