Admissions / MDS Coordinator (RN) | Full-Time

We need an Admissions/MDS Coordinator to join our team at The Advanced Therapy Center at Chesterwood Village in West Chester, OH.$36 to $43 Per Hour - Registered Nurse (based on licensure and experience)PLUS we offer additional shift differentials for evening and weekend shifts!!You'll assess new and existing patients to ensure that all of their care needs are addressed and documented on the MDS. You'll Be Responsible For:Overseeing the MDS process following the RAI manualWorking with other MDS nurses to complete initial assessments for new admissionsProperly managing patient admissions scheduling, MDS assessment & care planningCompleting required submissions weekly or as directed by the DONReviewing validation report to ensure error-free submissionObtaining & reviewing QM'sWorking with IDT to improve resident outcomesAttending IDT Medicare & Medicaid meetings in collaboration with IDT to complete the MDS processAddressing the needs of residents by developing & updating comprehensive & nurse aide care plansEnsuring non-nursing sections of MDS & CAA's accurately reflect resident needsMonitoring care plans to identify resident needsReviewing order recap reports & nurses' notes to ensure care plans are followed & determine whether additional OBRA MDS assessments are neededMonitoring residents' change of status & determining need for change per RAIEnsuring accuracy of supportive documentation according to MDS/RAI regulationsWorking with facility consultants to implement recommended changesCompleting AIMS, Fall Risk scale, Braden, Progress note, Care Plan, nurse aide care plan (Kardex) paperworkYou'll Need To Have:A current RN license in the state of OhioA flexible work schedule & the ability to work independentlyAn understanding of the MDS process & a willingness to continually learnKnowledge in PDPM & RUGWe're Proud To Offer You:Paid Time OffPaid TrainingHealth InsuranceDental InsuranceVision InsuranceLife InsuranceDisability Insurance401K with Employer MatchFlexible ScheduleEducation ScholarshipsContinuing Education CreditsEmployee DiscountsExceptional Care & Quality Service are the hallmarks of Chesterwood's in-house team of talented and experienced licensed therapists, nurses and caregivers.Chesterwood Village is an EOE/M/F/D/V and Smoke-Free Workplace.
