Responsibilities

We have an opportunity for a PNP who will provide on-going medical management for acute/episodic, rehabilitation patients.

Offering pediatric patients a continuum of comprehensive post-acute medical care, inpatient rehabilitation, and outpatient rehabilitation following an acquired or traumatic brain injury, sudden accident, or illness, Nexus Health Systems is the national leader in physician-driven clinical neuroscience.

Our medical model of care supports our patients from the earliest stages throughout the entire recovery process, delivering a holistic rehabilitation approach extending beyond traditional physical, occupational, and speech therapies. Multi-disciplinary teams develop individualized treatment plans addressing interpersonal, behavioral, cognitive, physical, and emotional issues to ensure underlying challenges are identified making rehabilitation more successful.

About the PositionProvide ongoing medical management and acute interventions for patients in the post-acute care setting, working directly with pediatric nurse practitioners within a group setting to provide direct patient care to skilled nursing and assisted living facilities and rehab patients.Work as a member of the interdisciplinary team to effectively designate processes for meeting the medical and psychosocial needs of the resident and providing updates and evaluating medical treatment plans, medications, and other necessary care.Flexible schedule while doing what you love most, providing excellent care.About the Facility
