Summary A Recruitment/Relocation Incentive is authorized for a highly qualified candidate. Responsibilities VA Illiana Health Care System is currently seeking outstanding full-time Family Practice or Internal Medicine trained physicians to join our Community Based Primary Care Clinic team in our Peoria Clinic. This incumbent will be responsible to provide comprehensive care, under the Patient Aligned Care Team (PACT) model, to a panel of patients managed under PCMM guidelines. The incumbent is expected to be attentive to patient education, preventive care and be able to work well in an interdisciplinary team to provide high quality primary care. The incumbent will be responsible for providing comprehensive medical care by performing duties to include, but not limited to the below responsibilities: Manages chronic conditions, evaluates and treats inter-current medical conditions Collaborates with a Patient Aligned Care Team (PACT) to provide accessible, comprehensive, coordinated, continuous care to an assigned panel of patients Performs a variety of disease prevention related activities including gender and age specific screenings, mental health screenings, and health promotion activities. Screens for, evaluates and treats uncomplicated common mental health disorders, including depression and common addictions, with referrals made to mental health providers for further evaluation and treatment as appropriate Primary care physicians will have responsibility for supervision of or collaboration with primary care mid-level providers Physicians are expected to provide clinical oversight and to participate in peer review of other primary care providers through periodic chart reviews Primary care physicians will act as surrogates for absent primary care providers when needed, to include review of CPRS view alerts Primary care physicians will regular attend primary care staff meetings, departmental meetings and medical staff meetings Timely documentation (notes and encounter forms) is necessary in all areas in accordance with facility policy. All administrative work should be completed in a timely fashion Clinical reminders should be addressed as medically appropriate. Quality of care should meet professional standards at all times Work Schedule: Monday-Friday; 7:45 a.m. - 4:15 p.m. Financial Disclosure Report: Not required Education Debt Reduction Program (EDRP) Authorized: This position is eligible for EDRP, a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of your start date. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Requirements Conditions of Employment Qualifications Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. Completion of residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. Proficiency in spoken and written English. Physical requirements outlined below. Preferred Experience: Board Certified in Family Medicine or Internal Medicine Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-2 Physician Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Light lifting (under 15 lbs.); light carrying (under 15 lbs.); use of fingers, walking 4-8 hours; standing 4-8 hours; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and 20/40 in the other; hearing (aid permitted); working closely with others; working alone; emotional and mental stability required.
