Is empowered to use judgment in the decision-making process within their sphere of activity.Graduation from an accredited School of Pharmacy (BS or Doctor of Pharmacy) plus current license and registration to practice in New York State.
Minimum 3 years experience as a pharmacist in Hospital Pharmacy with accepted standards of practice.
PharmD or MS degree and/or completion of Pharmacy Residency Program may be used as a substitute for some experience.
Maintains professional competence through participation in continuing education programs and review of periodical literature.
