Geriatric Team
provides consultation, assessment and direct services to older adults residing in long term care facilities in Central Massachusetts and MetroWest. The focus is on the emotional, behavioral and late-life problems that affect overall functioning. As a team member we consult with avariety of health care professionals and family members to ensure comprehensive care.The therapist works both independently and as a member of an interdisciplinary team. This position offers the opportunity to join a team, of Licensed Social Workers, LMHCs, Advanced Practice Psychiatric Nurse and Psychiatry. CHL provides a supportive environment, Monday through Friday schedule with flexibility,and no on call after hours.Responsibilities:Provides diagnostic evaluations and psychotherapeutic treatment services to adults in nursing home facilities with behavioral health conditions through individual therapy and brief intervention, consultation with nursing home staff, consultation with family members and legal guardians/representatives, and psychiatric consultation.
Provides direct clinical treatment to adults in nursing home facilities through diagnostic evaluations and screenings, urgent assessment and intervention, treatment planning and assessment, individual therapy, and case consultation.
Participates in interdisciplinary diagnostic treatment and planning conferences
Consults with nursing home staff and insurance providers regarding clients, client services, and case management
Maintains client records and administrative records according to clinic policy.
Attends administrative staff meetings.
Attends in-service education, clinical supervision and professional meetings as required.
Perform other related job duties as assigned.

Qualifications:Clinician III:
Master's degree in a clinical course of study in behavioral health (social work, psychology, marriage and family therapy, counseling or related field) that meet the academic requirementsfor independent licensure in psychology, social work, marriage and family therapy, or mental health counseling in the Commonwealth of Massachusetts. Licensure as LMHC, LCSW or is actively working towards licensure in the Commonwealth of Massachusetts preferred.
Clinician IV:
Master's degree in a clinical course of study (social work, psychology, counseling or related field) and LICSW licensure or licensed Psychologist
Ability to perform diagnostic and risk assessments using current accepted tools and practices to determine medical necessity.
Ability to develop and implement treatment plans which are behaviorally oriented, measurable, and appropriate for the identified level of care.
Bilingual Spanish/English preferred.
Clinical expertise specific to adults in nursing home facilities with behavioral health conditions with an ability to engage clients effectively and work with other professionals and nursing home staff.
Current valid US-issued driver's license and a registered, inspected and insured automobile for work related purposes including travel between work sites.
