Job Summary:TriStar Centennial Medical Center s premier cardiovascular consultant group is seeking a certified acute care provider for a VAD coordinator role in Nashville, Tennessee.

Qualified Candidates:Acute care nurse practitioner or physician assistant certificationTennessee licensure or ability to obtainTwo years hospital experience in a cardiac or CV surgical ICUExperience caring for mechanical circulatory support and advanced heart failure patients is preferredFull time, 4 10-hour shiftsShared 24/7 call 1:4 with other coordinatorsJob Responsibilities:

Coordinates and participates in patient evaluation and selection for advanced therapiesCoordinates/leads patient selection and performance improvement meetingsOutpatient clinic visits, clinical management, and care coordination (shared call with 3 other coordinators)Inpatient discharge planning/care coordinationInpatient and outpatient education to VAD patients and their caregiversOversees/provides education to acute care staff and community resources (EMS, rehab, home health, etc.)Participates in the development and maintenance of VAD statistical databases and associated registries (INTERMACS)Equipment and supply management using online inventory toolJoint Commission certification preparednessInvolvement with community outreach on VAD safety measures and awarenessLiaison between surgeons, heart failure cardiologists, industry partners and patientsIncentive/Benefits Package:
