Job Overview:Pharmacist team members are responsible for dispensing medication and review of physicians orders for appropriateness and safety. Pharmacists provide consultation to other healthcare providers, patients and patient family members as necessary and participate in pediatric clinical pharmacy services. The pharmacist role serves as a leader and provides guidance to pharmacy technicians.Qualifications:Required:B.S. or Pharm.D. from an ACPE-accredited School of PharmacyRegistered Pharmacist in NJAHA BLS CertificationPreferred:Completion of a PGY1 residency program or experience equivalentPediatric pharmacy experienceBoard Certified in area of pediatric practice or equivalent experienceCertifications and Licenses Required:Registered Pharmacist in NJAHA BLS CertificationScheduling Requirements:Per DiemEssential Functions:Prepares and dispenses medication orders per provider order according to established policies, procedures and protocolsIdentifies, resolves and prevents drug-related problemsInterprets medication orders and transcribes them into the EMR accurately if necessaryMaintains accurate and complete patient medication profilesCompletes accurate calculations for pediatric drug therapy for compounding, dispensing and drug therapy monitoringInteracts with patients, family, and other health care providers as appropriate to assure continuity of services and to achieve desired patient outcomes through safe, timely, cost-effective drug therapyIdentifies and selects specific dosage forms as appropriate for the pediatric populationAssures compliance with organizational policy regarding medication managementAssures integrity of medication including visual review, adequate storage and expiration managementParticipates in the prevention of drug diversion per department and facility policy and procedureEnsures universal pharmaceutical waste and hazardous pharmaceutical waste policies and procedures are appropriately completedSuccessfully utilizes automated dispensing systemPerforms assigned nursing station inspections every other month as assignedDemonstrates effective direction, supervision and evaluation of technical staffConsistently collaborates with medical and hospital staff to provide an environment which assures the safety and well-being of pediatric patientsAccurately reports all patient care or medication errors, adverse events and near misses as soon as possible and documents appropriatelyConsistently uses error prevention processes, especially those identified with high risk procedures or medication delivery systemsConsistently demonstrates knowledge of the chain of command policy and applies in appropriate situationsConsistently adheres to all patient safety and medication management policies and proceduresAssesses, collaborates, and coordinates the patients discharge planning needs and readiness for discharge with other members of the health care team as necessaryCompletes mandatory online educational sessions in a timely mannerDemonstrates awareness and respect to the patient, family and/or significant other as an individual and family unit (identifies cultural and religious needs, language barriers, physical and psychological limitations, etc.)Ensures timely and accurate communication of patient information with all members of the health care teamConsistently complies with all hospital and department HIPAA polices and preservation of patient PHIService: Strives to meet expectations/needs of customers; responds to concerns with sense of urgency; seeks equitable solutions for unreasonable customer expectationsQuality/Ethics/Integrity: Seeks opportunities for improvement; effectively manages issues and change; pays attention to detail; follows instructions and protocol/procedures; adheres to company code of conduct; maintains integrity in all aspects of his/her workRelationships: Builds effective working relationships; finds and provides information to clarify situations; expands on original ideas; contributes own ideas; seeks stakeholder engagement to support ideas; clearly explains rationale behind ideas/goals; demonstrates sensitivity to differing cultural views and needs; meets attendance and punctuality requirementsAdaptability: Seeks to understand current and changes to work tasks/processes; treats change and new tasks/processes as opportunities for learning and personal growth; speaks positively about changes to others; quickly modifies behavior to deal effectively with changeBasic Skills: Able to complete basic pediatric pharmaceutical calculations along with basic mathematics and practice appropriate grammar (verbal and written)Tactical Ability: Able to solve problems using analytical thinkingPhysical DemandsRequires prolonged sitting, some bending, stooping and stretchingRequires eye-hand coordination and manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator and other office equipmentRequires normal range of hearing and eyesight to record, prepare and communicate reports. Requires occasional lifting of boxes or totes up to 50 poundsMay be exposed to toxic/ caustic/chemicals/detergentsWork is performed in an office, and in the pharmacy and hospital environmentWork may be stressful at times. Contact may involve dealing with angry or upset peopleStaff must remain flexible and available to provide staffing assistance for any/all disaster or emergency situationsWork Environment:May be subjected to noise and disagreeable odorsMoving mechanical parts are found in the work environmentUniversal precautions must be followed when working in any clinical area and especially when entering patient care areasChemotherapy and hazardous medication preparation requires some exposure to potentially harmful chemicals and medications.
