Job Title: Physical Therapist Job Specialty: Specialty Code 5006 Job Duration: 13 weeks Shift: 08:30 AM - 04:30 PM Guaranteed Hours: 0 Experience: 1 year of professional SNF experience License: Relevant state licensure required Certifications: Applicable certifications required Must-Have: Minimum 1 year of professional SNF experience Job Description: - Provide physical therapy services to patients in a skilled nursing facility.- Evaluate and assess patients' physical conditions to develop treatment plans.- Implement therapeutic exercises and techniques to improve mobility and strength.- Collaborate with healthcare team members to ensure comprehensive patient care.- Document patient progress and modify treatment plans as needed.- Educate patients and their families on exercises to perform independently for ongoing health improvement.- Adhere to safety protocols and maintain a clean and safe work environment.- Stay updated with the latest physical therapy techniques and practices.
