Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County.
Sun River Health is currently hiring a full-time Unit Supervisor to join our Queens Health Home site! Please note this role may require some travel in the vicinity!
The Unit Supervisor is responsible for the supervision of care management staff and the daily care management operations of the program. This includes supervision of Health Home staff, ensuring quality of care management services provided to clients, management of staff documentation requirements, and staff training. The Unit Supervisor will also ensure the provision of adequate clinical support to clinically complex clients and clients in crisis.
Essential Functions:Provide direct supervision to care managers to assist with the development of effective interventions and Care plans.Ensure staff in kept informed to Health Home policies and procedure as well as departmental priorities.Receive appropriate trainings in order to maintain clinical standards.Monitor Health Home compliance and quality assurance.Provide crisis intervention to clients in psychological distress.Complete enrollment and service data tracking reports monthly or as needed.Continually review the service delivery process to exceed customer expectations.Review and approve client documentation as assigned.Consistently exhibit core values of team work, open book management, advocacy and customer service.Participate in quality assurance and quality improvement activities.Conduct staff meetings, training and supervision.Occasional field work may be required as needed to achieve client outcomes and health home requirements.Performs other duties ad requested.Facilitation of bi-weekly Clinical Case Review with Care Managers.

Pay: $60,000.00 - $65,000.00 per year
