Curana Health is a provider of value-based primary care services for the senior living industry, including skilled nursing facilities, assisted & independent living communities, Memory Care units, and affordable senior housing sites. Our 1,000+ clinicians serve more than 1,500 senior living community partners across 33 states, and Curana participates in various innovative CMS programs (including owned-and-operated Accountable Care Organizations and Medicare Advantage plans). This position is ideal for a physician who appreciates a flexible work schedule and minimizing uncompensated clinical work. There is limited, or no call required, and the schedule is flexible but must be able to round, once weekly for a few hours in person.
Responsibilities include:
Manage new admissionsTake care of patient visits, rounding and completed notes based on agreed schedule with facility partners and patient day-to-day needsEnsure exceptional patient care in a post-acute care settingEngage with the multi-disciplinary care team in building for quality and patient care needsWe provide:
