Position Title: PT I Total number of positions needed: 1 REQUIRED:I CANNOT post without the ICIMS # in the system for the perm posting: _ 1. How long is the contract assignment? 13 weeks 2. Schedule of assignment hours? M - F, 8am - 5pm? 3. Will this candidate be visiting patients in care facilities or only in patient's homes? ALF'S AND HOMES 4. Will you want to hire the contractor at the end of the contract or is this truly a contract assignment? POSSIBLY (CL will direct on pricing) 6. What region are you in? Home health or Hospice? HH 7. What's creating the request? - REPLACEMENT FOR PT MOVING OUT OF STATE 8. To which location will this person report for assignment, full address, Including the OU (Location Code) Please confirm above location is correct and list any other territories that are to be covered by the candidates: 609 SOUTH NEW HOPE ROAD GASTONIA NC SUITE 200A 3221/J03 CANDIDATE WOULD NEED TO COVER CLEVELAND AND RUTHERFORD COUNTY PRIMARILY W/OVERFLOW /NEED TO MEET PRODUCTIVITY AS NEEDED TO GASTON AND LINCOLN COUNTIES 9. How many miles per week will the consultant travel? 500 WEEKLY, MINIMUM 100 PER DAY 10. What are the weekly units of productivity expectations? . What EMR systems will the consultant be using: HomeCare HomeBase, Oasis, Outlook, TEAMS 12. Will there be any on call? If so, how much?
