Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.

We are currently looking to add a new member to our Enhanced Care Management Team to provide comprehensive case management services and housing navigation services. We serve patients from most counties in Northern California.

Who You Are:

You are passionate about making a difference in the lives of individuals through case management. You are good at problem solving, have experience and knowledge of community resources, including housing for low income and disabled persons, and help people solve social determinants of health care problems.

Travel:

This role is office-based. Travel could comprise up to 30%-80% of your time, visiting a participant in their home within our service region.

Location:

Chico preferred serving Butte and Glenn and surrounding counties. This is not a remote role and requires you to work in our office in Chico.

Schedule:

This is a full-time role from Monday-Friday 8am to 5pm with an hour for lunch.

Duties and Responsibilities:

Work with a Care Management Team comprising care managers and who provide comprehensive case management servicesAssists the team with finding community resources needed to meet the participant's needs. This includes, but is not limited to, housing, medical equipment and supplies, access to primary care physician and specialists, access to food clothing and household goods, medical transportation, and access to caregivers. Assist participants with applications for services, such as health benefits and insurance coverage, IHSS, CalFresh, low-income housing and Waiver and managed care programs.Engages in advocacy efforts to ensure participants receive timely medical services to remain safe in their communityResponds timely to questions and enquiries from the Care Management Team, participants, and from clinical and administrative staff representing other agenciesProvides education to participants so they can better understand the benefits of services and community support programsDocuments consumer interaction and maintains timely reporting and documentation requirementsObserves participants behavior; reports significant changes to supervisor or team or medical provider, and documents appropriatelyEssential Requirements:

Program knowledge and experience with HUD and housing programsCurrent CPR certificationValid California Driver's LicenseEligible to be insured under our liability policy with a clean DMV reportYour own car, and proof of current auto insuranceCOVID vaccinated or have an exemptionPre-employment background check and fingerprinting complianceRequired Skills and Abilities:

Experience providing care management services and knowledge of low-income housing resourcesAbility to communication effectively in both writing and verbally with colleagues, supervisors and participantsAbility to work the community for face-to-face visits with the clients and in the office environmentKnowledge of basic problems and needs of persons diagnosed with a physical disability or emotional disturbanceKnowledge of community and supportive servicesSkilled in assessing and prioritizing multiple tasks, projects, and demandsSkilled in working within deadlines to complete projects and assignmentsAble to establish and maintain effective working relations with co-workers, and community providersAble to use a computer utilizing a variety of standard software (including an electronic medical record)Ability to work independently as well as a team memberPreferred Skills:

Bilingual skills are preferred, especially in languages such as Russian, Spanish, Hmong or PortugueseEducation in social work or related field such as gerontology, sociology or psychologyMedical knowledge acquired through experience or educationPhysical Requirements:

Conditional upon job offer is successfully passing a medical exam that tests amongst other thing the ability to move, visual acuity, and hearing acuity. Physical requirement for this role specifically include

Sits, stands, bends, lifts, walks, and moves intermittently during working hoursAble to lift to 35 pounds without assistanceAble to drive intermittently

Compensation and Benefits:

Pay range is between $22-$25 per hour. Exact compensation may vary based on skills, experience, and location.

You will be reimbursed for the mileage you travel from the office to participant's homes.

You will receive a monthly phone stipend.

On the first of the month following 60 days of employment you will be eligible for health insurance, dental insurance, vision insurance, life insurance, as well as Flexible Spending Account (FSA).

Leave accrues starting on your first day of employment, and you can take paid vacation, sick and holiday leave after 90 days.

We offer a 401(k) matching up to 4% after 1 year of employment and 1,000 hours worked.
