Primary Responsibilities:
Serve the
Southwestern ValleyComplete senior wellness visits in clinics, virtual, and SNF/ custodial settings to see 10-14 patients per day. May be asked to do home visits on occasionTravel to Optum outpatient clinics and independently contracted physician offices, as well as some home and SNF custodial visits, 1-2 locations per dayConduct comprehensive assessment, perform and discuss recommended screenings (such as QuantaFlo, Neurometrix, or point-of-care A1C), comprehensively document and code encounter, and conduct patient education and appropriate follow upWork a schedule of Monday - Friday, with occasional need for Saturday clinics

Required Qualifications:
Master's degree from a four-year college and/or a professional certification beyond a four-year collegeCertificate of completion from an accredited Advanced Practice Clinician ProgramUnrestricted California Advanced Practice Clinician licenseCurrent AANP/ANCC/NCCPA certificationCurrent California DEA certificate required prior to start dateFluent or proficient Medical SpanishEMR ProficientExcellent patient care and time management skillsAbility to work in a fast-paced environment, team player, adaptable to changing priorities
Preferred Qualifications:
Previous Risk Adjustment experiencePrevious Quality STAR/HEDIS experience1+ years of previous managed care setting experience
California Residents Only : Th e salary range for this role is $109,513 to $160,000 per year. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable .
