Recovery Case Manager

The Recovery Case Manager

(RCM) will meet with their clients regularly on a one-on-one basis. The RCM is the primary contact for a client's family, loved ones, and referral sources. They are responsible for the planning and implementation of clients aftercare. RCMs also serve to assist clients with their outside needs (e.g. court hearings, application for unemployment benefits, FMLA).

Responsibilities:

Help hold clients accountable for their actions in the community and the completion of their workFacilitate and provide timely documentationMonitor compliance on client contracts, phone policy, buddy relations. Ensure that all pertinent information is notated on RSS office board at shift changeAbility to work in a fast-paced, dynamic environmentAbility to communicate effectively in written, electronic, and oral formAbility to make sound decisions and/or find alternative solutionsMake weekly calls to the clients family (more communication may be required). This communication will be logged in the EMR. Phone calls will address client progress, behavior, contracts, growth opportunities, programming, and aftercare. RCM must be prepared to discuss enabling, codependency, and/or Al-Anon. RCM is responsible for scheduling all calls and face-to-face meetings with family members and/or loved onesFamily calls are to be within the first 24 hours of admissionReferral calls are to be made within the first 24 hours of admission. Calls to referral sources are to be made weeklyMeet with clients at least once a week to monitor the treatment processResponsible for executing step downs and dischargesUploading/delegating current ROIs and aftercare plans into EMRRCMs are expected to attend the following meetings: Morning Meeting, RCM Meeting, Supervision, when necessary, Safety Squad and Quality AssuranceClient engagement is not limited individual case loadIntroductions are to be made with new clients on caseload within 48 hours of admission; interaction must be logged in the EMRQualifications:

High school diploma or GED requiredExtensive experience within Mental Health and Substance Use Disorder industryTime management skills and the ability to work on multiple projects simultaneouslyCoordinate with all departments and oversees clients stay at GMTCCreation of an after-care plan that realistically meets the needs of the client



Vetting of client visitors and phone callsMust meet then maintain all pre-employment requirements which includes reference checks, applicable state and national required background screening, 10 panel urine drug screen, DMV check, insurance (if applicable), and license/credential verificationsBenefits:
