The Primary Care Physician's Role:

The Primary Care Physician (MD/DO) is a key member of our interdisciplinary care team. In the role, you will evaluate our patients' acute and chronic conditions, manage their multiple medications, mental health, socio-emotional needs, and provide empathetic patient education. The PCP will work closely with fellow clinical teammates to provide comprehensive primary care. While direct patient care is the primary focus of the position, our PCPs also assist with development, implementation and achievement of key organizational objectives and metrics. Functioning as a value-based care model, our team of clinicians proactively and strategically manage our population's health in accordance with IHI's Quintuple Aim, adopt best practices in coding compliance and achieve our HEDIS quality metrics.

Qualifications:Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from the 50 United States or an accredited foreign medical school with proof of ECFMG certificationBoard certified in Internal Medicine or Family Medicine; Fellowship in Geriatrics desirableCurrent, active physician (MD/DO) license from the Minnesota Board of Medical Practice or willingness to obtain upon hireActive DEA registration with controlled substances prescriptive authority within the state of Minnesota or willingness to obtain upon hireBasic Life Support (BLS) certification or willingness to obtainFamiliarity with population health and experience in achieving quality metricsExperience or knowledge in value-based care models desirableEligibility and completion of credentialing and registration with Medicare and private payersEmployment dependent on passage of background check and drug screeningEligibility to work in the USAbility to learn and efficiently utilize technology including EMR, Microsoft Suite, etc
