As Intake Coordinator,

you'll have a dynamic and critical role

facilitating the admissions process for our Supportive Services for Veteran Families Program. Our programs supports the well-being and stability of Veterans and their families who are homeless, experiencing housing instability As Intake Coordinator you will assess and evaluate clients to screen for qualifications and needs for additional services. In addition to client assessment and assignment, the Intake Coordinator will foster collaborative relationships with internal staff and external partners to ensure a seamless experience for clients.

Intake Coordinator Key Responsibilities:Serve as Point of Contact for all SSVF referrals to identify initial eligibility based on veteran status and income within VOAMASS SSVF service area.Assign potential participant to Outreach Specialist(s) for pre-screening and needs assignment to determine complete eligibility of SSVF enrollment.Audit pre-screened client files to ensure accurate data recording is conducted.Coordinate with Program Manager(s) to discuss veterans available for case management assignment.Maintain understanding of all VOAMASS veteran services programs to facilitate inter-agency client referralsConduct regular meetings with Outreach staff to case conference and review any policy changes.Refer clients who do not qualify for the SSVF program to other community services for assistance, or to other housing stabilization programs in their service area when they do not meet eligibility.Communicate to potential participants the process of the SSVF program and required documentation to proceed with program participationMaintain log of all veteran contact requesting SSVF services and track client determined to be ineligibleConduct Pre-Screen and Needs Assessment for time-sensitive cases and/or when Outreach staff is unavailableParticipate in all appropriate staff, supervision, and training meetings as needed for VOAMASS veteran services.The Intake Coordinator role is about making a difference in the lives of the people we serve. As an Intake Coordinator, you'll bring the following qualifications:

BA/AA degree preferred; however, a minimum of 5 years experience in a case management or related role may be substituted.Experience in clinical assessment, evaluation, targeted crisis management, treatment, and care planning of individuals with mental health, substance use, dual diagnosis, and co-occurring disorders.Knowledge of veteran issues, homelessness, and state & Federal resources highly preferred.Computer proficiency is preferred in electronic health records, Microsoft Word and Outlook.Demonstrated excellent written and communication skills.Since 1934, VOAMASS has provided critical services to the residents of the Commonwealth.
