Is aware of and follows all Individual and Program Specific Safety Plans and approved behavior program.•Ability to carefully monitor and supervise individuals served.•Is prepared for and actively participates in all mandatory meetings.•Demonstrate flexibility in meeting program needs•Respects the role and responsibility of his/her supervisor and maintains the same work diligence whether his/her supervisor is present or not.•Maintain a courteous, professional attitude while conducting business by effectively communicating with all LifeStream personnel, family members, and all community contacts.•Completes all assigned tasks in a timely manner with minimal assistance.•Maintain cleanliness of program areas•Avoids bringing personal matters to work that could have a disruptive impact on the consumer or co-workers.•Consistently advocates for the individuals served in all areas above.QUALIFICATIONS AND REQUIREMENTS:18 Years of ageValid Driver's licenseAuthorized to work in the USOnce hired, must attend LifeStream orientation and trainingsAccess to a vehicleEDUCATION and/or EXPERIENCE:Demonstrated history of quality work performance and teamwork in previous positionsHigh School Diploma or GED or CNA/HHAOne year experience in the field of Human Service
