We are currently seeking a highly organized and dedicated Clinical Intake Coordinator to join our team. This role is crucial in managing the intake process for new patients, ensuring a smooth transition to our care services.Position Summary:The Clinical Intake Coordinator is responsible for managing all incoming referrals, processing patient admissions, and coordinating care with our clinical team. The ideal candidate will have prior experience as a Licensed Practical Nurse (LPN) and Health Unit Coordinator (HUC), providing them with the clinical insight and administrative skills necessary to excel in this role.Schedule:This position offers two shift options: 8:00 AM - 5:00 PM or 10:00 AM - 7:00 PM, with a weekend rotation required.Key Responsibilities:Handle all incoming patient referrals and inquiries, ensuring timely and accurate data entry into the system.Collaborate with clinical and administrative teams to schedule and coordinate initial patient assessments.Verify insurance eligibility and ensure that all necessary documentation is complete and compliant.Communicate effectively with referral sources, patients, and families to explain services and answer questions.Maintain a working knowledge of home care regulations and patient care processes.Monitor the status of ongoing referrals and admissions, updating records and staff as needed.Provide administrative support to clinical staff to ensure smooth day-to-day operations.Qualifications:Active and unrestricted Licensed Practical Nurse (LPN) license required.Prior experience as a Health Unit Coordinator (HUC) is highly preferred.Strong knowledge of medical terminology, patient care procedures, and home care regulations.Excellent organizational skills and attention to detail.Ability to manage multiple tasks and work in a fast-paced environment.Proficient with electronic medical records (EMR) systems and Microsoft Office Suite.Strong interpersonal and communication skills, both verbal and written.Ability to work collaboratively with a diverse team of healthcare professionals.Benefits:Competitive salaryHealth, dental, and vision insurancePaid time off and holidays401(k) plan with company matchProfessional development opportunitiesApplicable MOS codesIf you're a military member transitioning to civilian life, we value your service and skills.
