Description of the Role:

Always Best Care Senior Services - Oahu is currently seeking a skilled and compassionate Physical Therapist to join our team. As a Physical Therapist, you will provide exceptional care to patients in their homes, following physician orders. This is an intermittent position, allowing you to make a positive impact on a flexible basis.

Responsibilities:Conduct thorough assessments of patients' needs, both initially and ongoingCollaborate with physicians to develop and revise treatment plans based on patient progressAssist in the development and implementation of an interdisciplinary patient care plan, specifically related to Physical TherapyWork closely with physicians to apply diagnostic and prognostic tests to evaluate patients' muscle, nerve, joint, and functional abilitiesUtilize various physical therapy techniques, including exercise, massage, heat, water, light, and electricity, to relieve pain, improve function, and maintain peak performanceConsult and educate staff members on the established treatment planInstruct and guide patients and their families in the physical therapy programTeach patients how to properly use wheelchairs, braces, crutches, canes, and prosthetic and orthotic devicesProvide necessary equipment to support the treatment planCoordinate out-patient services for procedures that cannot be administered at homeMaintain accurate and timely records of evaluations, treatments, and patients' response to therapy interventionPrepare thorough assessments, evaluations, and summaries of patients' conditionsSupervise Physical Therapy Assistants and paraprofessionals, adhering to agency policiesReport any changes in patients' condition or reaction to treatment to the attending physiciansCommunicate effectively with physicians and other team members to ensure comprehensive patient careRecord detailed clinical and progress notes, incorporating them into patients' records in accordance with agency policiesParticipate actively in case conferences, physical therapy staff meetings, agency activities, and in-service programs as requiredOversee the work of Home Health Aides and Physical Therapy Assistants when physical therapy is the sole discipline involved in patient careContribute to discharge planning activities and complete Physical Therapy Discharge SummariesRequirements:Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A., in collaboration with the American Physical Therapy AssociationValid license to practice Physical Therapy in the state of HawaiiPreferred: One year of institutional/home health experienceCriminal background check requiredCurrent CPR certification
