The Parker Jewish Institute for Healthcare and Rehabilitation, located in New Hyde Park, NY, is seeking a

Full Time Nurse Practitioner or Physician Assistant

to join our team of dedicated specialists working in the Medical Department.

In this role working alongside caring, dedicated, and experienced Medical leadership, the Nurse Practitioner/Physician Assistant will report to the Senior Vice President of Medical Services, or the supervisory Attending Physician, and is responsible for the delivery of compassionate and quality medical care of assigned residents in coordination with other members of the health care team.

Examine residents and deliver quality care in varied settings to include subacute, long term, rehab and outpatient careOrder diagnostic workups and tests and timely completion of medical records and discharge summaries in a timely mannerAssist in resident/patient/family education and re-examine, re-evaluate and modify care plans as necessary, including maintaining proper documentation in resident chartsPrescribe medication as necessaryReview all diagnostic testing results and make recommendations to findingsEnsure Encounter forms are completed accurately and in a timely mannerEducate and supervisor rotating medical and physician assistant studentsParticipate in on call monthly/weekend medical coverageMaintain continual medical education creditsPosition Qualifications

Two (2) or more years of experience in a sub-acute or long term care facility administering medical care to residentsGraduate of an accredited physician assistant or nurse practitioner programClean and active license as an NP or PA New York State, with prescriptive privilegesProficient in SigmaCare or another EMR (Electronic Medical Record) softwarePrior experience with the geriatric population a plusFully vaccinated against COVID-19
