What You'll Do:Resident Care Coordinator | RCM

Manage care giving staff including hiring, training, evaluations and performance managementDevelop and post schedule ensuring adequate daily staffing. Communicate with staff regarding meetings, trainings, etc.Be available for after-hours staffing concernsCoordinate care team orientation and trainings with HR ManagerAssist in coordinating the daily administrative and care giving functions of the Community under the direction of the Executive DirectorOrganize supplies and materials, prepare work orders for maintenance department as requiredManage care team for completion of housekeeping dutiesAssist nursing staff in communication with residents, families, physicians, and other outside healthcare providersAssist with the maintenance of resident recordsSupervise the personal care of residents and assist in monitoring the wellbeing and the environment of the residentsReview and understand all resident individual service plans. Assist in ensuring that resident service plans accurately reflect resident care needs and are updated as neededEstablish and maintain positive and professional working relationships with residents, and all team membersMaintain knowledge of current OSHA and state regulations and routinely monitors Community compliance with the regulations related to resident care and nursing functionsObserves residents for changes in physical, emotional, mental, and social condition, unusual symptoms, accidents, injuries, or unusual occurrences, and reports promptly to a licensed nurse or Resident CoordinatorCompletes all forms such as incident reports, and written and computerized documentation, as necessary for communication ofresident information with other shifts, Resident Coordinators, and with the licensed nursesEnsure that changes in resident condition are monitored, documented, effective interventions are implemented, and reported appropriately to nurse’s physician and/ or family membersWhat We're Looking For:Resident Care Coordinator | RCM

CNA, or previous experience working with the elderly in a residential or long-term care setting preferredCurrent CNA license, other license, or ability to obtain as required by State regulatory agenciesMust enjoy working with seniorsAbility to establish effective relationships with residents, family members and team membersMust be able to read, comprehend, write, and speak English to communicate effectively with residents, families, other team members and physiciansMust be able to communicate effectively with residents, families, team members, visitors, physicians, etc.Strong documentation skillsStrong interpersonal skillsMust have basic computer skills, including Word, Excel and OutlookAbility to develop systems for monitoring resident health conditions and for ensuring efficient and safe delivery of resident careStrong organizational skills and ability to re-prioritize daily tasks to accommodate fluctuating needs of the residents and the communityAbility to represent the community in a positive and professional mannerMust be able to meet all health-related requirements pertaining to state licensureMust maintain current CPR/First Aid and Food Handlers credentialsMust maintain annual Continuing Education CreditsWhat We Offer:
