Primary Care Physician - Central Coast, CA Job Description: Pacific Central Coast Health Centers is seeking dedicated and experienced Primary Care Providers to join our team in Morro Bay or Santa Maria, California.
Responsibilities:
Provide comprehensive primary care services, including preventive care, acute illness management, and chronic disease management.Conduct thorough medical assessments, perform physical examinations, order diagnostic tests, and interpret results to develop appropriate treatment plans.Collaborate with patients to set health goals, educate them about their conditions, and empower them to take an active role in their healthcare journey.Coordinate care with specialists, allied health professionals, and community resources as needed to ensure continuity of care and optimal patient outcomes.Maintain accurate and up-to-date medical records, including patient histories, progress notes, and treatment plans, in compliance with regulatory standards.Participate in quality improvement initiatives, peer review activities, and continuing education to enhance clinical skills and knowledge.Requirements:
