Prepares medication orders by admixing medications under the supervision of a physician, calculating correct dosage volumesChecks all orders for completeness of information, verifies patient information and proper physician authorizationChecks all orders for insurance approval before admixingProperly disposes all materials used when mixing chemo or non-chemo IVs. Cleans hood daily or as stated in policy and proceduresLogs and dates all drugs being used. Checks drugs for monthly outdate and other quality assurance activities as assigned.Maintains proper control documentation of medications. Completes inventory control of medicationsPerforms efficient data entry into systems used by admixture.Orders medication and supplies as directedQualifications
High school diploma or equivalent requiredThree to five years experience in a pharmacy with IV skills preferredIV certification strongly preferredExperience with Lynx, Epic, Pyxis, Omnicell is preferredCertified Pharmacy Technician certificate or State Pharmacy Board preferredWilling to travel between offices as needed
