St. Lawrence,Jefferson, & Lewis County New York . This is a Full-Time position and allowing you to work alternating in home and virtual setting.Please review the below job description and Apply Now if you are interested.Nurse Evaluators will be working in the community conducting strength-based assessments for families who have children (ages 0-21) with behavioral health/psychiatric issues. The Nurse Evaluator will conduct on average 3 assessments per week and work with a Family Support Coordinator to meet the family's case management needs. This position is hybrid.The anticipated wage for this position is between $50 and $55. Hourly wage may depend upon experience, education, geographic location, and other factors.JOB RESPONSIBILITIESDevelop Home and Community Based Services (HCBS) Plan of Care (POC) in collaboration with child and family that includes evaluation of health status.Perform initial in-home evaluations of children that includes evaluation of health status, strengths, care needs, and preferences and guide the development of individualized long-term care service plans.Review child's medical documentation and or health referral forms, as relevant to the caseEmphasize continuity of care, thus reducing or eliminating fragmentation, duplication, and gaps in treatment planDiscuss with child/family health care options, supports needed, service vendor options and waiver options.Update plans of care as per State requirements, and on an as needed basisRequired Qualifications:2 + years of relevant experienceGraduate of an accredited nursing programLicensed in NYS as an RNPreferred Qualifications:BilingualPediatric experienceBehavioral Health experienceWhy work for Adecco?Weekly Pay401(k) PlanSkills TrainingExcellent medical, dental, and vision benefitsBenefit offerings include medical, dental, vision, life insurance, short-term disability, additional voluntary benefits, EAP program, commuter benefits and 401K plan.
