The

Intake and Discharge Coordinator

facilitates the intake, discharge, and housing navigation of residents temporarily housed in Transitional Housing programs. This position reports to the Intake and Discharge Program Manager.

Essential Job Functions

Acts as the point person for the day-to day operations Transitional Housing Intake and Discharge System; ensures smooth coordination of daily intake process receiving programs and property management.Responsible for the fidelity of the reporting system and tracking tools re: master list, HMIS and E.H.R.Utilizes personal phone or provided work cell if needed to field referrals from multiple points of entry.Understands contractual requirements and works with Program Manager to ensure Intake/Discharge Operations are meeting all targets.Identifies goals and strategies to better conduct integrated-care and/or VA intake procedures, better serving the needs of homeless individuals.In collaboration with Program Manager, communicates all process changes in Intake/Discharge arena to other CCH Programs, Partner Agencies, Funders, and other community stakeholders.Partners with Program Manager to complete data delivery to the appropriate funding, such as City of Denver, MDHI, VA and other investors and community partners.Provides reports and updates to CCH Leadership as requested.Provides ongoing housing case management and navigation services in conjunction with case management staff.Performs other duties as assigned.Qualifications Summary

Bachelor's degree or relevant experience in a related field required. Experience may be substituted.Experience serving homeless community or public health sector preferred.Excellent organization skills and willingness to work flexibly under pressure.Must have a valid driver's license.

$23.22 - $27.85 an hour
