Job Summary:

CASES is seeking a Licensed Practical Nurse (LPN0 to join the he Forensic Assertive Community Treatment (FACT) program, The FACT is designed to help adults aged 18 and older with serious mental illnesses (SMI) who havent been adequately served by traditional methods. These individuals often have co-occurring substance use disorders and a history of involvement with the criminal legal system.

Assertive Community Treatment (ACT) is an evidence-based practice that offers an integrated suite of services, including evidence-based treatments, rehabilitation, case management, and support. A mobile, multi-disciplinary mental health treatment team delivers these services, providing a highly individualized approach to support recipient recovery. The goal is to empower recipients to obtain and maintain housing, employment, and healthy relationships while managing symptoms and medication side effects.

Within the FACT program, the Licensed Practical Nurse (LPN) plays a vital role. Working under the guidance of the teams Registered Nurse (RN) and/or Prescriber, the LPN provides medication management, administration, health education, counseling, and psychoeducation. At least 80% of their services are delivered in the community, adhering to ACT core principles, professional nursing ethics, and regulatory guidelines. The LPNs focus is on promoting recovery, wellness, and optimal health for this vulnerable population.

Salary:

The salary for this role is $36.26 per hour.

Shift Hours:

This position is full-time/ Monday through Friday 9:00 am to 5:00 pm

This position is On-site: This role is 100% in-office/in person.

What you will be doing:

Provides at least 80% of medication management and health services to consumers in the community, including but not limited to: medications administration (both oral and intramuscular), monitoring medications side effects and adherence, phlebotomy, coordination of consumers’ health care services, providing psychoeducation to consumers about health and medications, assessing consumers’ mental statuses;Completes and signs progress notes, incident reports, and other required documentation in accordance to the New York State Office of Mental Health (OMH) regulations, the New York City Department of Health and Mental Hygiene (DOHMH) guidelines, and Medicaid billing standards and regulations;Coordinate with FACT RN and prescriber on consumers’ medication management needs, health care services, maintaining consumers’ medication administration schedules;Coordinates care with family and other community service providers or collaterals including, but not limited to: hospitals, housing providers, shelter staff, criminal justice personnel, Assisted Outpatient Treatment (AOT), and primary health care providers;Participates in daily team meetings, clinical meetings, and monthly nurses meetings to collaborate and discuss clinical issues and clients’ needs with team members and F/ACT nursing staff;Use motivational interviewing, culturally and developmentally appropriate, recovery oriented and trauma-informed approaches when delivering services;Any other duties as required by Team Leader and senior clinical leadership.

What we are looking for:

Licensed Practical Nurse (LPN) with current New York State licenseAt least three years of experience working with adults (ages 18 and up) in one or more of the following areas: psychiatric nursing, substance use treatment setting, social service setting(s) caring for individuals experiencing homelessness, justice involvement or limited social support;Knowledge of community treatment, support services, and resourcesHighly organized and self-motivatedProficient in computers

Preferred not recommended:

Fluency in Spanish preferred
