Home Care Field / Visiting RN seeing patients in their homes M-F 5x per week.
What you will be doing
Complete initial assessments, OASIS, recertificationsCreates and implements a plan of careCollaborates with the interdisciplinary team and coordinates patient careEducates patients and families
Skills you need
RN license in NYHome Care Experienced Required as a RN, LPN, CNA, or HHA.
