Requirements :

CA License

1 year of experience with knowledge of post-acute levels such as Home Health, Hospice, Palliative Care, SNF, LTAC, B&C, Sub-Acute and Acute rehab

EPIC experience

AHA BLS

Job Description:

The RN Case Manager is responsible for Care Coordination, Care Transitions, and Discharge Planning throughout the acute care patient experience. Responsibilities include completing timely Initial Assessments, updated notes, and final notes, as well as referrals to post-acute levels of care. The RN will coordinate with the multidisciplinary team to ensure the discharge is on track.

Shift : 4 x 8 Days Fri-Mon, no on call required
