We are a home health and hospice agency providing services in patient homes in 22 counties throughout Wisconsin. Patient homes include private residences, assisted living facilities, memory care units, and resident care apartment complexes. Preceptor strives to provide high quality care through our multifaceted programs including home health, hospice, palliative care, and outpatient services. We have branch offices located in Germantown, Fond du Lac, Waukesha, and Appleton.
We are currently in need of a
Full-Time
Home Health Registered Nurse Case Manager
for our Washington County territory.
Full-Time, 32-40 hours per week.
SIGN-ON BONUS OF UP TO $5,000 PAID IN INSTALLMENTS
Minimal On Call is Required!!
Job Requirements:Carry a current Registered Nurse license in the State of WisconsinExcellent communication skills with patients, families, facility staff, doctors, and teammatesAbility to work independentlyGood organization and effective time management skillsWillingness to travel to meet patient needsCompliance with documentation standards and regulationsPrior home health experience preferred, but does not affect eligibility to applyAdditional Information:
