Job Summary:We are seeking a dedicated and skilled Physical Therapist to join our rehabilitation team. The successful candidate will assess, plan, and implement individualized rehabilitative programs to improve patient mobility, alleviate pain, and enhance overall physical function. The position requires collaboration with patients, their families, and healthcare professionals to deliver high-quality care while adhering to regulatory standards. This role also involves providing clinical supervision to assistants and aides and serving as a clinical instructor for students.
Key Responsibilities:
Rehabilitation Services:
Develop and execute personalized treatment plans aimed at improving or restoring physical function, reducing pain, and preventing physical dysfunction in patients.Conduct patient evaluations and diagnostic tests in compliance with established standards and regulatory requirements.Formulate and update individualized Plans of Care (POC) based on evaluation data, identified issues, and diagnostic results.Deliver and adjust treatment interventions as per the POC, ensuring standards of care are met.Document all treatments and procedures accurately for charge capture in accordance with departmental standards.Coordinate patient care effectively with other healthcare providers and team members to ensure optimal outcomes.Maintain flexibility in shift assignments, including varying days and hours of work as needed.Provide clinical supervision to physical therapy assistants and aides.Serve as a clinical instructor for assigned physical therapy students.Clerical Duties:
Accurately document all patient-related therapy activities in medical and departmental records as per regulatory standards.Perform general administrative tasks, such as answering phones, updating patient schedules, and managing electronic health records (EHR).Safety:
Participate in quality improvement initiatives and interdisciplinary projects to enhance service delivery.Maintain a clean and safe working environment, ensuring all equipment is disinfected after each use.Inspect and operate equipment according to manufacturers' guidelines, reporting any malfunctions as required.Compliance:
Ensure the strict confidentiality of all patient Protected Health Information (PHI) and comply with all HIPAA regulations.Document and deliver services in accordance with state and federal regulations.Maintain active certification and complete all continuing education requirements to comply with facility accreditation and licensure standards.Education:
