Nurse Practitioner

to support our client’s medical facility based in

New York, NY.

This is a

7 to 12 months assignment

opportunity and a great way to start working with a top-tier healthcare organization!Responsibilities:Conduct comprehensive assessmentsEffectively manage acute and chronic medical and behavioral conditionsEnsure accurate and complete documentation, including ICD 10 conditionsCommunicate and collaborate with the interdisciplinary care teamConduct advanced illness and advanced care planning conversationsProvide patients and caregivers with counseling and educationEducation/Requirements:Active and unrestricted license in the state which you reside, or ability to obtain by start dateNP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice programPA: Graduate of an accredited Physician Assistant degree programActive Nurse Practitioner or Physician Assistant certification through a national board:NP: Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following:Family Nurse PractitionerAdult Nurse PractitionerGerontology Nurse PractitionerAdult-Gerontology Acute Care Nurse PractitionerPA: Board certified by the National Commission on Certification of Physician AssistantsCurrent, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice).Ability to complete physical requirements of the job including lifting a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, and sit, stand, and kneel to perform physical assessmentAccess to reliable transportationAbility to gain a collaborative practice agreement, if applicable in your stateFull COVID-19 vaccination is an essential requirement of this role.Preferred Qualifications:1+ year of clinical experience in practice (long-term care setting preferred)Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needsUnderstanding of Geriatrics and Chronic IllnessUnderstanding of Advanced Illness and end of life discussionsProficient computer skills including the ability to document medical information with written and electronic medical records.Job Title : Nurse PractitionerLocation : New York City, NYSchedule : Mon to Fri 8 am to 5 pmPay Range : $65 to $70/hr (Rate open for negotiation)Candidate will travel to BronxHow many facilities will you be traveling between:
