The Ideal Home Health HH Physical Therapist PT will have a minimum of 1-year experience in the geriatric population. Must have at least one year of experience as a Home Health HH Physical Therapist PT, although new grads are encouraged to apply. This can be a travel or a local assignment.Responsibilities of a Home Health HH Physical Therapist PT: "Traveling to different home care settings Creating individualized treatment plans that can be carried out at home Outlining treatment goals and methods to be used Assessing patients and monitoring their progress Using techniques like functional training, exercise, medication, diet changes, and specialized equipment Meet all productivity standards and requirements daily Must have a clean driving record to apply 40 hours guaranteed 13-week assignment Submit all billing logs and necessary information on a timely basis New grads and experienced therapists are encouraged to apply - We will support you through this transition"Pioneer Healthcare Services
