They collaborate with healthcare providers, monitor patient progress, and ensure a smooth transition between different stages of care well advocating for the patients' well-being.What You Will Do
As a Registered Nurse RN Case Manager - Emergency Department, you will serve as a driver of a collaborative, process which coordinates, monitors, and plans health care services throughout the patients’ illness.
Minimum QualificationsEducation: Bachelors of Science in Nursing BSN degreeLicensure: Current Licensure as a registered nurse RN in ConnecticutExperience: Five years of diversified, progressive experience in acute care and/or other settings within the continuum of Care including an understanding of nationally recognized medical utilization criteria, HCFA/OBRA and state of Connecticut regulatory compliance issues, and JCAHO accreditation accountabilities.Certification: BLS certification required
