The RN practices professional nursing as a registered nurse within the legal and ethical framework established by the Connecticut Nurse Practice Act, American Nurses Association Scope and Standards for Nursing Practice, ANA Code of Ethics for Nurses and the YNHHS Professional Practice Model. Responsibilities

The Mission Control RN is responsible for providing high-quality patient care to various patient populations within the framework of the YNHHS Home Hospital Model and includes the following key elements:

Utilizing monitoring, assessment, planning, intervention and critical thinking skills to care for acute care patient populations in the homeNavigating patients and families across the continuum of the YNHHS Home Hospital phases of careMonitoring and responding to biometric data per YNHHS Home Hospital protocolsConducting home and audio/visual patient care visits as directed by the care team and/or protocolEnsuring all interventions in the patient’s care plan are implemented efficiently with a patient centered approachLeveraging the resources and expertise of the YNHHS Home Hospital team and supplier partners to implement collaborative workflows, standards, policies, protocols, guidelines and documentation systems to support safe, reliable, high-quality, evidence-based care with clinical protocols as the foundationAdhering to the clinical protocols and be willing to obtain the necessary training to provide care within the context of the providing evidence-based carePromoting professional practice and a culture of safety; willingness to engage in process improvement efforts

Clinical Roles And Responsibilities

Providing protocol driven assessment, care coordination and interventions as appropriate informed by YNHHS Home Hospital protocols for diverse patient populations with a focus on oncology, medicine and post- operative careExhibiting sound clinical decision making and critical thinking skillsMaintain and practice in accordance with nursing guidelines per state and hospital regulationsFacilitating admissions to YNHHS Home Hospital utilizing our guidelinesServing as a role model for professional nursing practiceUtilizing nursing knowledge to identify, prevent and/or solve complex acute and/or recurring patient care problemsSharing expertise with the multidisciplinary team and serve as a resource to staff in area(s) of expertiseSupporting research efforts to advance knowledge and promote evidence-based practicePreforming other duties and responsibilities as assigned by the Director of Nursing

Quality And Safety Responsibilities

Ensure compliance with reporting requirements in RL Solutions for Sentinel Events and Serious IncidentsParticipate in quality and safety investigations and root cause analysesTake appropriate action to ensure patient safety as a result of any investigation; andServe as a resource in quality improvement processes and technique

Qualifications

EDUCATION

Graduation from an accredited school of nursing. BSN preferred. All newly hired registered nurses must have current BCLS certification or must obtain within one month of hire.

Experience

Recent 2 years bedside nursing required. Care Management experience a plus Excellent communications, interpersonal, coordinative, and organizational skills.

ACCOUNTABILITY

Provides nursing care to an individual or group of patients; recognizes changes in patients condition and reports such changes to Physician and/or Patient Services Manager., Utilizes and manages material resources in a cost effective manner.

COMPLEXITY

Plans, develops and implements patient care plans; evaluates the effectiveness of existing patient care; evaluates the effectiveness of existing patient care plan and modifies plan of care to meet patient needs, Please see RCAP CMP for further details.

LICENSE

RN license and a currently registered nurse in the State of Connecticut.

All newly hired Registered Nurses must have current BCLS certification or must obtain within one month of hire.
