This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. The Clinical Command Center (CCC) Nurse Practitioner at the VA Palo Alto Health Care System (VAPAHCS) provides direct patient care and works collaboratively with members of the Clinical Command Center and Surgical Service specialties to ensure optimization of patient flow throughout the health care system. Areas of responsibility encompass assisting with clinical evaluations, consultation, and collaboration with patients, families, nursing staff, physicians, multidisciplinary staff, and other healthcare team members regarding the surgical care and needs of patients with acute and chronic surgical disorders. Major duties and responsibilities include, but are not limited to: working collaboratively with the specialty Surgery teams on the inpatient services and the Clinical Command Center, participating in daily interdisciplinary team meetings, engaging in STEPS Huddles, utilization management review process, assisting with transfers into and out of facility, and may participate in rounds of the surgery and interventional procedural teams on inpatients. Assist with coverage of medical teams and their patients. Ensures urgent/emergent patient care issues are addressed quickly with attending surgeon/fellow/chief resident and arranges admissions/transfers as needed. Works in collaboration with a wide range of interdisciplinary staff to collect data and participate in improvement projects related to Surgical flow, Clinical Command Center, and Acute Care metrics. Under the direction of a surgeon, the candidate performs: obtains complete and/or interval histories and performs pertinent physical exams with assessment of normal and abnormal findings on hospitalized patients. Orders appropriate lab tests, radiology, and nuclear medicine studies, and assists with the follow-up of those orders and documents treatment plans. Obtains outside medical records needed for patient care. Initiates consults to other medical and surgical services, social work, palliative care, and hospice. Consults, collaborates, and serves as a clinical liaison with patients, physicians, nursing staff, UM team, and other interdisciplinary health team members regarding the comprehensive needs of Surgical inpatients and their clinical management. Coordinates appropriate peri-operative care and collaborates with Case Management, Social Work, and Clinical Command Center for discharge planning needs and posthospital care. Will notify supervising physician regarding patients with atypical findings or a problem that is not resolving as anticipated. Educates patients and their families as needed in anticipation of discharge. Performs all other duties as assigned. Benefits
