Job Details Job Description
The Virtual Primary Care Family Medicine Advanced Practice Provider (Nurse Practitioner/Physician Assistant) provides comprehensive primary care to new and established patients for Jefferson Health via telehealth platform. This service will expand Jefferson Health primary care services to new and existing markets. S/he collaborates with, and is an integral member of, a multidisciplinary health care team.
Responsibilities:
Assessment/Diagnosis and Management of Patients in Primary Care Setting
Provides patient-centered, preventive health, including well visits, screenings, and immunizations recommendations; as well as acute and complex care visits requiring diagnosis, treatment/management.
Collaborates with the interdisciplinary health care team.
Obtains and documents thorough patient history.
Orders, performs, and interprets appropriate diagnostic testing, and responds to results requiring further evaluation or investigation.
Formulates differential diagnoses by priority.
Coordinates with and refers to specialists and/or a higher level of care as needed.
Completes professional billing in timely and accurate manner, in compliance with regulations.
Other Responsibilities:
Reviews results of diagnostic studies in a timely manner, and communicates follow-up plan to the patient.
Maintains ongoing communication and collaboration with office staff and interdisciplinary health care team.
Qualifications:
Master’s Degree
National Board Certification as a Primary/Family Nurse Practitioner or Physician Assistant
Minimum two years of Primary Care experience required
Licensure or eligibility for licensure in PA and NJ required
BLS certification
Telehealth/telemedicine experience preferred, but not required
