Job Description DEPARTMENT: Residential POSITION TITLE: Case Manager - THP FLSA: Non-Exempt SUPERVISOR: Program Manager Job Code: NA SALARY: $25-$27 The Case Manager for Minors and Non-Minor Dependents in the Transitional Housing Program (THP) provides direct support and intensive case management services to assist Transitional Aged Youth (TAY) aged 16 to 21 years, helping them meet individualized goals and connect with community resources. The typical caseload is 12 youth. This role involves collaboration and coordination within an interdisciplinary team. As with all UC staff, this position requires adherence to the performance standards outlined in the UC Staff Employee policies and procedures. DIRECT REPORTS: None QUALIFICATIONS: Required: Bachelor's Degree in areas such as Marriage, Family and Child Counseling, Child Psychology, Child Development, Counseling Psychology, Social Psychology, Clinical Psychology, Educational Psychology, Education with Counseling emphasis, Social Work, or Social Welfare, including content for licensure as a Licensed Professional Clinical Counselor. OR a Bachelor's Degree in Human Services or Behavioral Science with two (2) years of experience in child welfare social services working with TAY aged 16–24. OR CCL approval as an Excepted Case Manager prior to start date. Additional required skills include experience working with the TAY population, knowledge of Microsoft Office applications, case work and coordination skills, understanding of transformational care planning, and excellent interpersonal skills. Preferred: Master’s Degree in relevant fields as listed above. Two (2) years’ experience working with at-risk youth or adolescents in a residential setting. Bi-Lingual in Spanish and English. RESPONSIBILITIES INCLUDE (but not limited to): Program Implementation and Client Services: Establish relationships, facilitate engagement, maintain contact, support independent living skills, and coordinate care with team members. Compliance and Reporting: Complete reports timely, maintain accurate records, ensure regulatory compliance, and conduct audits. Program Productivity: Meet productivity standards, maintain documentation, and evaluate clients appropriately. Program Ambassador: Serve as community liaison, develop resources, and coordinate with external agencies. Organization Duties: Submit timecards timely, follow policies, and perform other duties as assigned. Essential Duties and Qualifications: Must be able to interpret documents, communicate effectively, solve problems, and perform physical tasks including lifting up to 25 pounds. Certifications in CPR and First Aid are required within 10 days of start, along with background checks, health screenings, and valid driver’s license with insurance.
