ASAP Assignment Length:
Ongoing (Contract to Hire) Schedule:
Part Time Hours:
12pm to -5pm (Monday to Wednesday)
and
8am to 12pm (Fridays) Patient Population:
Adults and Families with mental health and substance abuse challenges Responsibilities: Assist in the completion of assessments and plans for each person in the home, obtaining, developing and recording the information necessary for the completion in person or remotely Complete intake assessments during the referral process and review current & historical records and documentation Perform direct observations and interviews with the individual and their support group to determine support needs and services required for success. Participate in the transitional planning process working closely with the Qualified Behavior Modification Professional (QBMP), including Individual Education Plan (IEP), Individual Program Plan (IPP), Individual Behavior Support Plan (IBSP) meetings and seek out community resources that will best support individual needs Perform other duties, as needed Qualifications: Master Social Worker in California Solid assessment and documentation skills Excellent communication skills Desired Skills: Knowledge of California's Lanterman Act and its related service system, Title 17 and/or Title 22 regulations
