CVHCare is looking for a qualified Physical Therapist. Assigned county Territory is Area #6 Vacaville, Fairfield, Rio Vista, Suisun City, Dixon and Area #7 Vallejo, Benicia, Napa, American Canyon, Yountville. Additional travel outside area may be required. Weekend coverage may be required. Job Requirements
Current CA PT License BLS Drivers License Auto insurance Auto Registration TB (annual) MMR Varicella HepB TDap (10 years) Flu (annual) Drug screen Physical Schedule Information
If Guarantee, the following shall apply: Productivity: At least 27 points per week. 36 hours Guarantee. Traveler must complete 30 points in the week. If Non-Guarantee, the following shall apply: To secure a full caseload, Traveler may be required to go outside the service area occasionally. Additional Details
Your Agency's contract to your traveler MUST mirror the confirmation page. All Requested Time Off (RTO) must be disclosed at the time of submission.
