Overview
We are looking for a
Care Transitions Clinical
to join our team. This position will directly report to the VP Care Coordination and is responsible for supporting the Director of Care Transitions in the proactive identification of patients potentially suitable for hospice care. Additionally, this role involves tracking these patients to ensure optimal continuity of care during their transition into our services.
Assist with care coordination and will not conduct patient assessments, patient care planning or discharge planning
Communicate with the Director Care Transitions and help facilitate timely patient follow-up
Adhere to and participate in Company’s mandatory training which includes but is not limited to HIPAA privacy program/practices, Business Ethics and Compliance programs/practices, and Company policies and procedures
Review medical records against nationally recognized clinical criteria guidelines using clinical decision making to determine the eligibly for other levels of care with DCT oversite and review
The shift is Mon-Fri 3:30pm-12am EST.
About You
LPN or RN Nursing Degree
Licensed Practical/Vocational Nurse licensed in the state of employment; RN preferred.
May be required to obtain licensure in additional states as dictated by business needs.
Nursing experience in post-acute care
Good working knowledge of home health, hospice and palliative care services
Excellent knowledge of state and federal home health/hospice agency benefits, eligibility, regulations/conditions of participation and compliance standards and requirements
